PRIVILEGE CARD APPLICATION FORM

1. The card remains the property of Pantiles and can only be used when making purchases from the authorised centre named on the front of the card.
2. Subject to the terms hereof, reward vouchers will only be sent to UK resident cardholders who have fully completed and returned this application form.
3. To qualify for reward vouchers the card must be presented at the time of purchase.
4. Reward vouchers cannot be redeemed against gift vouchers, bottled gas and they carry no cash value. Change cannot be given against reward vouchers.
5. Reward vouchers redeemed for goods sold in (concessions) or franchises, at coffee shops and restaurant is left to the discretion of the nursery.
6. Pantiles reserves the right to decline issue, withdraw the card or alter its conditions at anytime, or terminate the card facility without notice.
7. It is the responsibility of the cardholder to notify the nursery of any change of address or personal details.
8. You may apply for a new card if the card is lost stolen or damaged. Any accrued benefit will be transferred to your new card.
9. If the cardholder does not use their card in a twelve month period the holder will be deemed to have withdrawn from the scheme and unredeemed benefits will be withdrawn.
10. A cardholder’s card and any unredeemed accrued benefits are not transferable.
11. A cardholder’s card cannot be used in conjunction with any other offer or promotion unless agreed to by the nursery.
12. The conditions of use stated on the reverse of the card form part of these “Conditions of Use”.
13. The card is not a credit payment or cheque guarantee card.
14. Nothing in these “Conditions of Use” shall affect your statutory rights.
15. All details provided on the card remain confidential and are not passed to any outside companies.
